Once your first appointment is scheduled, you will receive a link to complete your new patient forms electronically. We recommend completing these forms prior to your visit so our providers can review your information and make the most of your appointment time.
Booking online?
If you schedule your first appointment through our online store, you’ll have the opportunity to complete your required patient forms during the booking process.
What to Expect
Your first visit is all about you. Depending on the service you’ve selected, your provider will review your health history, discuss your concerns and goals, answer your questions, and recommend a personalized treatment or wellness plan designed specifically for you.
Please arrive with your forms completed whenever possible and bring a current photo ID, a list of medications or supplements you are taking, and any information that may be relevant to your treatment.
